Registered Nurse Manager - Home Care Program Director - Gainesville, Florida
Duties


The Home Based Primary Care Program Director is to report directly to ACOS/GEC or equivalent. Management functions include, but are not limited to: planning, staffing, directing, budgeting, evaluating, and tracking the program, and developing VA and community relationships. Resource management responsibilities include assuring appropriate resources are designated for the support of the HomeCare Programs to include Home Based Primary Care (HBPC), and safety of the HomeCare/HBPC staff. These include appropriate space, time, medical and information technology, and communications equipment, as well as vehicles for the daily use of HomeCare/HBPC staff.

Directing the clinical services offered by the program to ensure that the program is in compliance with local and national VHA standards and policies as well as accreditation standards for home care organizations. The development and continued effective functioning of the interdisciplinary health care team. The Home Care Program Director recognizes and effectively utilizes the skills, knowledge, and contributions of each team member.
Collaborating with other HBPC Program Directors, MFH Programs, VDC Programs, HomeCare leadership team, VISN leadership, and VA Central Office staff on issues of program development and operation.

Major duties and responsibilities include, but are not limited to:

• 
  
• Communicating VA policies to the HomeCare/HBPC team, VA medical facility staff, Patient Aligned Care Teams, and non-VA entities.
  
• Developing and implementing the local HomeCare/HBPC Standard Operating Procedures (SOPs), to operationalize the hospital policies, with input as indicated from interdisciplinary service line managers or chiefs.
  
• Managing program SOP and activities to include, but not limited to, performance improvement, patient safety, utilization review, emergency preparedness, and staff safety.
  
• Evaluating the effectiveness of the HomeCare/HBPC Program (e.g., program outcomes, performance improvement).
  
• Managing human resource items for the HomeCare/HBPC Program including: selecting qualified staff in collaboration with the HomeCare Medical Directors, HomeCare leadership team, and Administrative Officer, orienting and mentoring the staff, and promoting staff development and professional growth to ensure competency.
  
• Coordinating training and educational programs.
  
• Acquiring, managing, and remaining accountable for programs' resources and resource utilization.
  
• Processing, assigning, and monitoring referrals of Home Based Primary Care Programs to ensure timely access and care coordination.
  
• Managing consult process for Home Based Primary Care Programs' referrals, in collaboration with the HBPC consult team, as necessary.
  
• Collaborating with appropriate data and technology departments to ensure accurate HomeCare/HBPC clinical, administrative, and workload data.
  
• Providing supervision to clinical patient care and administrative Home Based Primary Care Program staff.
  
• Other duties as assigned by the ACOS/Geriatrics & Extended Care.
  
• The Nurse Manager is responsible for developing, implementing and routine evaluation of all administrative, clinical and operational aspects of the program.
