Job Description:

A RN Complex Case Manager is needed to work in a Transitions of Care model, assessing, planning, implementing, and monitoring comprehensive care plans for patients.





The RN Complex Case Manager will:

--- Collaborate with medical team members, including physicians, social workers, Case Management, PT/OT staff, pharmacists, respiratory, dietary, and nursing staff to identify discharge barriers and complex patients needing focused care management.
--- Provide Care Transitions for Florida Blue patients with high risk for readmission, working with post-acute care teams to prevent 30-day readmissions.
--- Educate patients on disease management and medication, including special recommendations for discharge to social workers and Case Managers.
--- Work with outpatient Care Transitions teams to provide handoff and care plans for FLBL patients.





**Requirements:**

--- Licensed RN in Florida
--- BSN preferred
--- 2 years recent RN experience applicable to the designated population served
--- Prior Case Management experience preferred
--- Professional certification in specialty area preferred





**Note:** Case Managers hired after January 1, 2018 will be required to obtain a BSN and/or MSN within three years of employment, and specialty certification within two years of employment.
