Behavioral Health Nurse Advocate RN - Remote, Field Work, Tampa

work.(sm) - The NorthStar Behavioral Health Advocate (BHA) works in
partnership with other members of the Interdisciplinary Care Team
(ICT) to provide clinical support focused on closing medical and
behavioral gaps in care. In this role, you will provide clinical
insights to address member specific concerns, identify needs that
require escalation to the Medical Director for review, and provide
treatment recommendations focused on improving the member's quality
of life and community tenure. Your behavioral and medical expertise
will be essential to your success in this role and your ability to
impact the lives of our members. - - This is a remote position that
includes up to 25% field work in the local community. The schedule
is Monday through Friday, 8am - 5pm. - If you are located in or
near Gainesville or Tampa, FL, you will have the flexibility to
work remotely* as you take on some tough challenges. Residency in
one of these two areas is required due to business needs. - Primary
Responsibilities:
- Collaborate with a multidisciplinary ICT to develop care plans,
provide treatment and provider level insights, and connect members
to community-based providers based on their individual needs,
preference, and objectives
- Assist with team development of Motivational Interviewing
techniques to help members identify and understand intrinsic goals
and engage in intentional behavior change to meet their goals
- Provide education and consultation to a multidisciplinary team
on medical, behavioral, and ancillary services available to members
to address complex and comorbid health needs -
- Develop relationships with local consumer-operated
organizations to engage difficult to reach members
- Serve as member of internal and/or external treatment and care
teams as needed or requested to facilitate access to and transition
between services and providers
- Provide support to members, caregivers, or authorized
representatives to schedule and/or attend appointments as
needed
- Provide support to, and advocate on behalf of, members,
caregivers, and families while working with internal matrix
partners, community agencies, and providers to ensure effective
collaboration and improved clinical outcomes that are aligned with

Required Qualifications:
- Bachelor's degree
- Licensed Registered Nurse (RN) in the State of Florida with
Behavioral Health experience
- 3+ years of experience providing direct clinical care in a
medical and/or behavioral health treatment setting with emphasis on
assessment, case management, and crisis intervention
- Dual diagnosis experience with medical and behavioral
health
- Community behavioral health experience
- Demonstrated knowledge of Medicaid benefits
- Ability to listen, collect relevant information, build rapport
quickly, respond compassionately, work independently, adapt to
change, and interact with internal and external customers in a
variety of environments
- Access to install secure, high-speed internet and a dedicated,
distraction-free workspace at home
- Reliable transportation and the ability to travel as needed
within the designated service delivery area (up to 25%)
- Reside in or near Gainesville or Tampa, FL
- Full COVID-19 vaccination is an essential job function of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

Qualifications:
- Inpatient psychiatric experience
- Intermediate level of proficiency with Microsoft Office (Word,
Excel, and Outlook)
- Bilingual in Spanish - If the hired individual resides in
Florida (office based or telecommuting) this position requires the
AHCA Level II background check (fingerprinting) by the State of
Florida for all clinicians that have direct face to face contact
with members OR employees who will have access to confidential
patient data and will require renewal every five years. - - To
