Job SummaryThe Clinical Social Worker provides comprehensive psychosocial services to patients, assessing their needs and developing plans to address them. This role requires collaboration with the healthcare team to ensure seamless care coordination.Key Responsibilities

• Assess patients' psychosocial needs and develop individualized plans to address them
• Collaborate with the healthcare team to ensure effective care coordination
• Provide crisis intervention and advocacy services as needed
• Work with patients to identify and access community resources
• Document patient interactions and progress in a timely and accurate mannerRequirements
  • Master's degree in Social Work (MSW) from an accredited institution
  • License-eligible for LCSW in the State of Florida
  • Professional certification in specialty area preferred
