Job SummaryThe Clinical Social Worker provides and coordinates psychosocial and behavioral services to patients in a designated setting. This role assesses, plans, implements, and monitors a plan of care in collaboration with the medical team, including physicians, nurses, case managers, and pharmacists.Key Responsibilities

• Works with the care team to identify patient needs and advocates for patients when possible.
• Provides direct services, resources, and counseling to patients to ensure a safe and effective transition/discharge from the hospital or clinic.
• Assesses and responds to crisis intervention situations, including potential abuse/neglect of vulnerable patients.QualificationsA Master's degree in Social Work (MSW) is required from an institution accredited by the Council on Social Work Education. Professional certification in a specialty area is preferred, along with recent clinical experience applicable to the designated population served. Experience in a healthcare setting is also preferred.Motor Vehicle Operator DesignationEmployees in this position will not operate vehicles for an assigned business purpose.
