Job Title: Lead Case Manager Profession: RN Specialty: Case Management Duration: 10 Shift: 08:00-16:30 Hours per Shift: 8 Experience: Minimum of 2 years of social work or case management experience in an inpatient setting preferred. License: Current Registered Nurse or Social Work licensure or a Master's or Bachelor's in Social Work or healthcare professional licensure. Certifications: Certification in Case Management or Rehabilitation preferred, such as: - Commission for Case Manager Certification (CCM) - Association of Rehabilitation Nurses (ARN) certification - American Case Management Association (ACM) - Board Certification in Case Management by the ANCC Must-Have: Basic computer knowledge Effective communication skills in English, both verbal and written Description: The role is dedicated to the treatment and recovery of individuals experiencing severe injury or illness. Rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey. Customized and individualized rehabilitation is offered, tailored to the specific needs of those recovering from various conditions.
