Job Description:
As a leading healthcare services provider, HCA Healthcare is
seeking a dedicated RN Case Manager Lead to join our dynamic team.
In this role, you will be overseeing the case management process,
ensuring efficient patient care management that adheres to the
company’s standards and regulatory requirements. The ideal
candidate will coordinate healthcare services across different
settings, working closely with healthcare professionals to advocate
for patient needs, optimize treatment plans, and reduce healthcare
costs while maintaining high-quality standards. Lead and oversee
the case management team, implementing best practices and improving
current processes. Evaluate patient care plans to ensure
effectiveness and compliance with all regulations and standards.
Coordinate with healthcare providers to streamline services and
optimize patient outcomes. Provide training and support to case
management staff to enhance their professional competences. Monitor
patient progress and make necessary adjustments to care plans.
Conduct regular meetings with healthcare teams to discuss patient
treatments and challenges. Maintain accurate records and reports
related to case management activities. Strong leadership and team
management skills Excellent organizational and multi-tasking skills
Proficient in healthcare management systems and applications
Effective communication and interpersonal skills Critical thinking
and problem-solving capabilities Knowledge of healthcare
