Drug Free Workplace

The School Board of Levy County policy requires drug testing. After being offered employment and prior to reporting to your worksite, the Personnel Department will contact you to schedule your drug test within 24 hours. You will need to report to the CareHere Center, 445 Ishie Avenue, Bronson,FL 32621 at your scheduled time. Failure to make your appointment or a positive test will result in the job offer being rescinded. Please take positive photo identification with you. At the lab, you will be asked to provide a urine sample. The SBLC will be billed and pay for the test. The lab's doctors will notify you of any positive test results, as well as notifying our office. We may have a conference with you to discuss positive test results and you will be notified in writing of the results. I hereby affirm that I have received this information on SBLC policy regarding drug testing. I understand that employment is contingent upon reporting to the lab for drug testing within 24 hours of a job offer and a negative test result.

Conditions of Employment

* Are you a U.S. citizen or otherwise legally authorized to work in the U.S.? * Are you willing to submit to a drug test? * Are you willing to submit to a Level 2 Criminal Background check? * In accordance with FS 1012.315 (, you will not quality for FL certification if you have been convicted of certain felony offenses. Can you confirm that you have never been convicted of offenses such as sexual misconduct, aggravated assault or aggravated battery? * In accordance with FS 1012.315 (, you will not quality for FL certification if you have been convicted of certain misdemeanor offenses. Can you confirm that you have never been convicted of a misdemeanor such as battery on a minor or luring or enticing a child? * Do you understand that any false or misleading information in this application (or any omissions of information) shall be full and sufficient grounds to refuse to employ or having been employed shall be immediate cause for dismissal?

Full Time emloyees (25 hours per week)

Board Pays for Employee Only:
