This position is in a 24-hour Medical Residential Facility.

The primary function of this position is to provide direct supports and services to residents requiring 24-hour medical care in the implementation of their individual active treatment plans as prescribed by their Interdisciplinary Team (IDT) while maintaining their overall safety and well-being.

Assists residents with completion of their routine self-care needs and daily living activities, such as personal hygiene, dressing, leisure activities, and social activities and community outings. Escort residents to medical and therapy appointments

Assist IDT members with the completion of comprehensive functional assessment for prioritizing individualized plan of care.

Accurately documents resident information including, but not limited to behavioral issues, toileting, food/fluid intake, and active treatment following each activity as required. Provides valuable input concerning the observation of the resident and makes suggestions for improvements in the activities under discussion.

This is a full-time position. We offer various shifts and regular days off.

Requirements:

A valid Florida CNA license

KSAs:

Ability to lift 40 pounds.

Knowledge of the methods of instructing and providing care to others.

Ability to read.

Ability to pay attention to details.

Ability to follow verbal and written instructions.

Ability to carry out instructions for implementing prescribed treatment plans including behavior modification activities.

Ability to instruct others in basic self-care skills.

Ability to instruct others in recreational activities.

Ability to provide instruction and personal guidance and care to residents.

Ability to communicate effectively verbally and in writing.

Ability to establish and maintain effective working relationships with others.
